Practitioner agreement
By applying to and maintaining a listing on the Wellness Registry, you agree to the following.
You are an independent contractor
You are not an employee, agent, or representative of the Wellness Registry, and the Registry does not supervise, direct, or control how you deliver your services.
Credentials and compliance
You confirm that all credentials and training listed are accurate and current, that you hold any licenses required in your jurisdiction, and that you comply with applicable laws and professional standards.
Accurate representation
Your profile must accurately describe your training, scope of practice, and the nature of your work. You may not make medical claims, guarantee outcomes, or misrepresent your credentials.
Listings and fees
Approved listings begin with a 90-day free trial with no payment collected. Practitioners who subscribe before December 31, 2026 lock in the founding member rate — $19/month or $190/year — for the life of their subscription. After that date, the standard rate applies: $29/month or $290/year.
Billing and cancellation
Subscriptions renew automatically each billing period until cancelled. Cancellation takes effect at the end of the current billing period, and all subscription fees are non-refundable. Resubscribing after December 31, 2026 uses standard pricing, not the founding rate.
Removal
The Wellness Registry may remove a listing at any time for violations of these terms or our inclusion standards. Practitioners may also request removal of their own listing at any time.
